How to Use CARE Notes

  4. Save with descriptive name (like "Cold-August-2025")
  5. Add follow-up notes to track progress over time

  • Document while it's happening - memory fades quickly during illness
  • If necessary, share with providers - Medical history is the most powerful diagnostic tool in clinical medicine, enabling accurate diagnosis in 75-85% of cases across studies spanning several decades. While physical examinations (8-12% of cases) and diagnostic tests (9-13% of cases) provide valuable confirmation and additional insights, the patient's history alone typically contains the key information needed to reach the correct diagnosis.
